No pattern to when it does or doesn't fire. I do notice though, that when there is a failure to fire, that the internal mag moves slightly, as if something hit it. I don't know if it's just me, but I feel like my SKS is way more accurate than other aks I've used. I feel the exact same way, this is why I never really get butthurt when somebody wants to try and modernize an SKS instead of just buying an AK.
